By clicking JOIN, I acknowledge and agree to the following:

1) I am agreeing to receive services over televideo

2) I am in a private location where no one else can hear our conversation. This protects privacy of my services and the services of others (if attending group therapy)

3) I can refuse to participate in televideo services at any time, and my Solvista Health team will work to help me find available alternatives

4) My provider will enter information into my electronic health record the same way they do if I see them in person. I have the same rights to get a copy of my medical records created during a televideo appointment that I do for records created in person.

5) Solvista Health’s system works on a special network that is private, encrypted, and secure. We do not record the conversation. I am just as safe talking to the provider over our video system as I am talking with them in the same room

6) Data rates may apply. By using the service you are responsible for understanding those rates and any charges incurred
